ROLES AND RESPONSIBILITIES:
A. Direct patient care:
· Report to the Supervisor/In-charge on duty.
· Be punctual for the duties.
· Complete special assignments like Inventory checking, dressing trolley maintenance, crash cart inventory etc.
· Take handover from the previous shift, check and take over the instruments and equipment.
· Attend doctor’s rounds & follow the orders.
· Follow specific PICU/NICU policies & guidelines.
· Get clean the equipment’s as per hospital policy.
· Up on admission / Transfer in of patients ensure readiness of unit, receive the patient and make him / her comfortable.
· Nurses should know the protocol on admission / discharge criteria’s.
· Pediatric nurse must have the knowledge & skill on Initial assessment / re assessment policies.
· Monitor patient’s vitals such as blood pressure heart rate and breathing and initiative action on an immediate basis.
· Meet the personal hygiene of the patients, including bathing, care of mouth, back, nails, hair etc.
· Ensure effective communicate with doctors & other team members.
· Nurses should have the knowledge on types of pediatric medications, dosage calculation etc.
· Pediatric nurse are expected to have a thorough knowledge about nonverbal signs of pain and recognizing its intensity by using appropriate pain assessment tools.
· Provide calming influence to children.
· Ensure close observation and individualized care based on the assessment & implement the care.
· Ensure Care of pressures points as needed
· Ensure to use appropriate diversionary therapy, be friendly to eliminate fear & to get better co-operation.
· Ensure Critical Thinking: Nurses must rely on critical thinking skills to quickly assess situations involving newborns.
· Ensure to respond immediately to emergencies and deteriorating situations. Nurses must remain ever vigilant, observant and detail-oriented to offer the best care to their tiny patients.
· Be Empathetic and Understanding to patients and relatives.
· Respond quickly to emergencies codes.
· All staff should know about all emergency codes & policies.
B. Infection control Practices
· Ensure the cleanliness of unit & patient premises at all times.
· Infection control protocols to be carried out, like Proper Hand hygiene, wearing PPE appropriately, Use of hand sanitizer before & after touching the patients & proper biomedical waste segregation standards, restricted entries to the sterile areas of NICU/ PICU.
· Attend HIC training sessions & update the knowledge.
· Mention date and time of insertion of tubes & catheters.
· Assess Patient’s IV Line frequently to prevent extravasations/ infiltration etc.
· To strictly adhere to implementation of buddle care check during the care such as CLABSI, CAUTI, VAP etc.
C. Training & Development :
· Attend 6 -10 hours of CNE training sessions every month.
· Attend departmental hands on training.
· Attend specialize training session on Child abduction policy (Code pink) and all other emergency codes.
· Ensure to attend training on age specific competency and skills.
· Nurses should have knowledge & skill on usage of all types of medical equipment used in the PICU/ NICU such as: ventilators, High Flow Oxygen, Incubators Resuscitation equipment for the new-born at birth Photo therapy unit Pulse oximeter C-PAP device Infant Warmer Cyclometer etc.
· Ensure to attend NICU/ PICU specialization trainings as per the schedule.
· Be proactive to clarify doubts to senior nurses, supervisors, Nurse educator etc. if there is any clarification required about the doctors’ orders, procedures, medication orders etc.
· Take initiative to do case study presentations as per the assignment given by the educator.
· Attend regular workshops/ CNE sessions related to IV Cannulations of paediatrics & Neonates.
· Nurses should undergo IV calculation training for Pediatric / Newborn babies.
· Pediatric nurses should know about immunization standards.
· Need to undergo specific courses on NALS & PALS
D. Medication Management
· Indent the medication as per the written order.
· Check patient’s case file and give medication –
· Right drug
· Right dose
· Right patient
· Right time
· Right route
· Right to refuse
· Right assessment
· Right Education
· Right Evaluation
· Right drug documentation
· Enter in the medication chart immediately after administering the drug and observe for any reaction
· Telephonic orders can be taken only at the time of emergency & it must be confirmed by another staff nurse or duty doctor and documented in the verbal order form and get it signed by consultant within 24hrs.
· Blood & blood products to have double check before transfusion.
