RN Case Manager Job Description

As an RN Case Manager, you will play a vital role in coordinating the care of patients from admission to discharge. You will assess patient needs, develop and implement care plans, and collaborate with healthcare providers to ensure the best outcomes. Additionally, you will provide patient education, facilitate communication between the patient, family, and healthcare team, and advocate for the patient's needs and preferences.

Responsibilities

Qualifications
