Case Manager Job Description
As a Case Manager, you will be responsible for assessing patients' needs, developing care plans, and coordinating services to address those needs. You will collaborate with healthcare providers, social workers, and community organizations to ensure that patients receive the appropriate level of care. Additionally, you will monitor patients' progress, provide education and counseling, and assist with discharge planning.
Responsibilities
- Conduct assessments to evaluate patients' medical, social, and emotional needs
- Develop individualized care plans in collaboration with healthcare professionals
- Coordinate and arrange necessary services and resources for patients
- Advocate for patients' rights and ensure access to appropriate healthcare services
- Monitor patients' progress and make adjustments to care plans as needed
- Provide education and counseling to patients and their families
- Assist with discharge planning and ensure smooth transitions of care
- Maintain accurate and up-to-date documentation of patient interactions and interventions
Qualifications
- Bachelor's degree in social work, nursing, or a related field (Master's degree preferred)
- Previous experience in case management or a related role
