Qualifications and Skills

• Proven expertise in care planning for geriatric patients, ensuring personalized and comprehensive healthcare solutions (Mandatory skill).
• Experience in chronic disease management specifically related to older adult populations, including multi-morbidity care approaches (Mandatory skill).
• In-depth knowledge and practical management of geriatric syndromes, such as frailty, falls, and dementia (Mandatory skill).
• Strong ability to conduct thorough patient assessments, including physical, cognitive, and psychosocial evaluations to guide treatment planning.
• Proficient in medication management for elderly patients, with attention to polypharmacy risks and medication reconciliation.
• Hands-on skill with electronic health records to effectively document, track, and manage patient information securely and accurately.
• Excellent communication and interpersonal skills to collaborate with healthcare teams and build trust with elderly patients and families.

Roles and Responsibilities

• Develop and implement individualized care plans for geriatric patients, addressing both medical and psychosocial needs.
• Monitor, diagnose, and manage chronic diseases prevalent among elderly populations, using evidence-based protocols and multidisciplinary approaches.
• Assess, identify, and manage geriatric syndromes, ensuring timely intervention and follow-up care.
• Perform comprehensive patient assessments to inform and adjust care strategies regularly.
• Oversee and optimize medication management, minimizing risks and ensuring best outcomes for aging patients.
• Accurately document all patient interactions, treatments, and progress using electronic health record systems.
• Work collaboratively with physicians, nurses, and allied health professionals to ensure cohesive and holistic patient care.
• Educate patients and their families on disease management, prevention strategies, and available health resources.
