We currently have openings for ‘Case Manager- Nursing - Home Care’, details for the same are given below

Candidates Requirement: -

·        B. SC Degree in Nursing from a recognized nursing

·        Experience – 7 to 10 yrs

·        Should have Nursing council registration

·        Minimum 1 to 2 years of clinical experience in the relevant area

·        Able to fluently communicate in English, Hindi

Job Profile: -

Deliver safe and quality nursing care in conformance with the Sir HN RFH policies and processes.

Ensure patient safety at all times according to hospital policies.

Ensure that patient privacy and confidentiality is duly protected at all times.

Treat patients, their family and other members of Healthcare team with courtesy, respect, and caring behaviour.

Take immediate action to resolve any issues, complaints or distress expressed in any way by the patient or their family with in her limits and escalate to higher level

Perform pre-assessment of patients basis their current and past medical records and advise on necessary services that can be safely delivered at non-Hospital setting/Home setting.

Perform patient’s assessment on service confirmation & as per changes in clinical condition and create/modify care plan accordingly.

Take inputs from family, treating Physician and care provider while creating care plan.

Communicate and monitor service providers at patients Home for delivery of updated care plan.

Ensure that service provider is instructed & monitored to deliver safe care and handling of patients with invasive lines and tubes.

Prioritize consultation of patients with clinical problem which warrant an earlier response through virtual /in person mode.

Identify early warning signs & respond as per the protocol and advise appropriate actions by the service provider in communication with patients family/next of kin and primary Physician in loop.

Provide guidance for transportation needs in case the patient condition warrants transfer to healthcare facility.

Ensure safety of patient by adhering to policies of patient identification, fall prevention, safe medication management, prevention of infection & biomedical waste management.

Raise incident in case there is non-compliance that impact patient safety/staff safety in non-hospital setting.

Use effective communication, written and verbal at all levels of staff including the multi-disciplinary team-in or out of the hospital, patients and relatives.

Ensure patient requests for Hospital @Home are within scope of home based/non-hospital setting.

Screen prescriptions for short term to ensure that the service can be provided within the scope.

Ensure that the follow up appointment and health education is given as per the protocol with respect to each patient by service providers as and when applicable.

Ensure that the care team members (Nurses, Physiotherapist, Doctors and all other applicable health team members) maintain accurate, comprehensive and timely medical records.

Work in coordination with Hospital @Home operations team /PE staff to ensure that the patient gets uniform experience and all queries are addressed. Use escalation matrix where required.

Identify training need and provide input to Manager responsible for training and development of the service provider staff for Hospital @Home.

Adhere to the safety norms for care to be delivered at Home/non-hospital settings.
